Family Connection Goals Template

The Family Connection Goals Template helps families write, display, and revisit one shared family goal at a time.

After using the Family Connection Goal Planning Guide or Family Connection Goals Tool to choose a realistic shared goal, families can add their completed plan to this clean, display-friendly template. They can post the finished page somewhere visible in the home, giving everyone a shared reminder of what the family is working on together.

This template gives families space to add the family or household name, the date started, dates revisited, the family goal, why the goal matters, what each person can do, how the family will check in, how progress will be celebrated, and when the goal will be revisited. Families can use it to stay focused on one manageable goal while practicing connection, teamwork, and shared success at home.

The Family Connection Goals Template does not focus on pressure, perfection, or expecting one person to fix the whole problem. Instead, it helps families keep one shared goal visible, realistic, and easy to return to. When families post the completed template where they can see it often, they can gently revisit their goal, notice progress, make adjustments, and keep practicing together over time.

This resource includes two printable goal templates, so families can begin with one goal and move on to a second goal when they feel ready. Each page offers a simple, practical format that families can use as part of everyday family life.

The Family Connection Goals Template is part of Connected Moments Parenting Series One, a relationship-centered collection that helps families build more calm, consistency, cooperation, and connection at home.
